From the photos I agree with 8 bit on it looking to be a sticker. Seems to be a clear edge outlined around the green. Also he seems to be missing an eye and some hair(bloom?)

I don't think that the character is missing any hair.
To me that looks like a Kappa, the Japanese mythological creature.
I have little to no knowledge about gameboy variants, but I guess that there could have been a Japanese one with a Kappa on it...

Couldn't think of the name of the little character for nothing. Circled is the part I was referring to as missing hair. If you look at the clear outline it is missing in that section. With Nintendo handheld variants anything is possible. Even if it is just a sticker it could be a variant.
So in summary, it's likely a sticker, although it could be an important sticker. However we will also likely never know if it is important.

Yeah variants aren't documented very well over the history of Nintendo handhelds. Even though anything is possible in the world of Gameboy I'm still leaning towards owner placed sticker over variant.br />

EDIT: Actually, since it's clear if you open it up and get alot of light onto it you should be able to tell if its a sticker.
