The first game I remember being scared by was the original Doom. Of course by today's standards, it's not that creepy, but I remember there being nothing else like it the first time I played it. Went through the same thing with the first Resident Evil....... damn dogs jumping through the window. I've hated RE dogs ever since.
Definitely Doom 3 and the Resurrection of Evil expansion disc. Kinda bloody but it can mess with your mind too with just the creepy sound effects too. You also get ports of Ultimate Doom, Doom 2 and Master Levels on the same disc as RoE.
Playing through Doom 3 right now, best way to spend October.
People knock Doom 3 but I really liked it. It has a great atmosphere. It's not for kids though..
I'm actually a bit sad that I got rid of this game years ago. It was a very unique experience. The subject matter gets a little disturbing if you think too deeply about it but overall it was a well-made game.

It's for Xbox too. Anyway, it's as violent as they come, but fun.

On the PC, give DoomRL or AliensRL a try. The sound effects really add to the atmosphere, and the games both kill you ridiculously easily.
About as scary/spooky as you can get with ASCII graphics.
