Yeah I can't lie...random battles can be a massive pain in the ass especially in a scenerio like in DW-II but on the other hand I do love overcoming a challenge!
I also hate the monsters in rpgs who spam the crap out of their special attacks or skills,like the mudmen/puppetmen in DW 2 who constantly do the strange jig to steal mp. And of course there's no way to defend it. Also the enemies who use dance or instant death spells are ultra annoying,especially when you come across a party of those type of monsters. T_T

Or monsters that permanently remove a party member from the battle entirely......ugh.
Everything in DW2 is annoying. That game contains copious amounts of player frustration-inducing BS. The only other RPG I remember having a major problem with insta-kill/remove from party junk was 7th Saga and the enemies that spam vacuum.
One thing I will say for DW2, though, is that it gave me tremendous satisfaction to finally limp into Rhone after getting destroyed at the very end of that hellish cave multiple times. I had one living party member with next to no HP remaining, and it actually allowed me to run from a couple of battles!

Haha! I remember limping into Rhone numerous times. Even when you are totally prepared for that cave it still has a knack for decimating your party.

Back to topic,I think these have been stated but I hate hunger meters (Wonder Boy,Adventure Island!) and the Gradius effect sucks a lot of dingus.
Yup, I remember DW2 being one of the most grindy RPGs I've ever played, and that final push before you're ready to enter Rhone was the worst part. I seem to remember getting thru the entire rest of the game world by level 23-ish, but needing to get to 30.

When I first played it,I was wondering where the level cap was so I grinded until I found it. I thought it was so strange that each character had a different cap.
You would think once your party is maxed out that the game would be a breeze (it is a lot easier) but all the brutal attacks/instant death spells/attacks do a good job keeping it from being the cake walk it should be.
Bumpin, playing thru Xenoblade again on 3DS and always found it annoying you have to use Enchant with your Monado just to allow the rest of your party to hit Mechon enemies. Find it lame it takes up a slot on your Monado when they could of gave you another sweet attack instead. Kind of a pain to have to cast everytime you enter a battle especially when you have to use regular attacks until your Monado's meter is even full, just to be able to cast the thing that the rest of the party needs to be able to attack. in Sword Valley now and theres nothing but Mechon enemies so i'm just venting i guess, lol. Still love the game tho!
Battle of Olympus and Super Tennis are two games I love, but their password systems are absolutely horrible. Both are extremely long and Battle of Olympus even commits the sin of using upper/lowercase letters and nice things like 0/O where you have a good chance of getting it wrong. It'll be much nicer using the AVS when it comes out!
I'll take another approach here and point out a game that is MISSING a feature that really pisses me off. Maximum Carnage on the SNES. Awesome beat-em-up game. WHY OH WHY is there no 2-player mode? You've got Spider-man, you've got Venom. Surely you and a pal can team up, right? Wrong. Seriously who the hell let that game release without that feature.

A good RPG, grinding for EXP should be fun, as combat isn't a chore. Combat should be fun with lots of options, or go by really quick and be nice and simple with entertaining animations, or some mix that works well. Slow combat where you just mash fight is teh sux.
one of the worst offenses for early shooters was not being able to invert the aim controls. I can think of Quake on the N64 that doesn't allow this. Overall, not being able to completely customize controls in any game is likely my biggest gripe with video games. Why the hell not?

QTEs, Medusa Heads and their derivatives, and having to level up skills that should be fundamental to your class. Example: having to level up meditate as a spell caster in EverQyest back in the day.
Also, having to stare at your spell book to meditate. /glares at EverQuest
