How about in Legend of Zelda in Gannon's dungeon.It's been a while since I've played it,but I know there's a certain room in that dungeon (top right corner I think) where you become trapped with no way of getting out and pretty much have to kill yourself or reset your game.Awesome.
Let me also add the watch item in Castlevania I and III. Some may disagree,but it has very minimal use to me.Not only that,but the holy water can be used to not only freeze enemies on contact,but it also damages them as well,which the watch doesn't do. It doesn't even fit in with the other weapons...dagger,boomerang,axe,holy water,and.....watch?!
Final Fantasy - Getting the Adamant to make the Masmune sword. The chest is rather far along in the fourth fiends dungeon, which is filled with the hardest enemies you have faced thus far. You than are required to backtrack out of the dungeon, than down the tower you climbed to get the the fiends dungeon, than through the desert you trek through to get to the tower to finally get to your airship so you can make the metal into the sword. Oh, and after all that, you gotta go back through the desert, up the tower, and back where you left to continue on.

what about items you pick up in Nintendo games that are just letters..I think there was a letter in Star Tropics...You pick it up and read it and it does nothing...The game came with a manual that had the letter glued to the back of it anyways? Do these items count? There are several other games with items that are just that..Useless...
I went with #10 power bracelet. Close behind were the pointless 1-up in Dash Galaxy and the unattainable pizza in TMNT. My rationale is that the Power Bracelet is under a totally random rock with no rhyme or reason, no clues, and you could seriously wander around forever not knowing where. And when you find it, what do you get? Access to a shortcut, which becomes totally obsolete once you get the whistle. Whoopee.
I'm inclined to agree with #2...fighting a blue Lynel (or attempting to do so) with 5 heart containers and most likely just the green tunic? You'd either be really good at running, or really good at dodging, because even if you're at full health, that's 2 hearts down the tubes for every hit. Plus, it has the sword beam attack, which travels faster than you can run away from it if you're at the top of the screen going around the north side of the pond there.
I don't know if submissions are still allowable but I would've gone with the M-card in the last level of M.C.Kids. It's directly over an enormous lava flow that kills you instantly. To get to the card, you have to navigate a series of conveyor platforms, find a way to turn yourself upside down, re-navigate the same conveyors back to the lava pit, make a leap of faith onto the block, then hope for the best as you fall UP and attempt to avoid a bunch of traps and then lava-kill on the ceiling. I blew through, like, 20 lives trying to get that last card.
