dear god... was the list ever posted? I'm wading through pages and pages....
And, more importantly: Does NA faithfully stick by this [to me, mythical til i find it] list when writing 'CIB', or is 'CIB' generically imply game, sleeve, manual, box?
This topic pops up pretty frequently. Definitions vary, but the most widely accepted is:
CIB - cart, instructions, box. Doesn't guarantee extras like styrofoam, dust sleeve, or random inserts. Some people consider maps (like in Legend of Zelda or the Dragon Warrior series) to be part of the instructions, others dont.
Complete - means everything that was originally in the box, including inserts, nintendo power ads, etc. Some people say 'complete' when they just mean CIB (happens on Ebay a lot).
Your best bet is to always ask for clarification because your definition may be different than someone else's and all that leads to is a "hey said, she said" situation where no one wins.

No, CIB generally means cart, instruction book, and box. You should always ask before buying, though.
