This Golden Axe is from my buddy Kelan's collection, this is the only time I have ever seen this manual variant before. This came in a non ESRB Classics case with a Mega Drive cart.
Another edit: The cardboard box varient of Sonic Spinball is a K-A release that has a black and white manual, which also says K-A on it. There is also a clamshell of the Sonic Spinball K-A release, which appears to still use the regular color-covered manual, but there's really no way to be sure how that clamshell K-A version was originally packaged.
Good info! I'll add that to my list. I think I already have like 3 SS variants, none of which are ESRB-rated. *sigh*....

None of them were pricey at all; it just takes a sharp eye while hunting in the wild. I can't really think of too many Genesis variants that actually demand a higher price than standard releases (Version 1 of RoS is about all that comes to mind), though I have a feeling that some of the more obscure stuff will climb as time marches on.

Has anyone mentioned the Revenge of Shinobi revisions yet? That game has so many variants it's out of control. Batman, Godzilla revisions and then the classics. It was released as a classics version with a color manual then released again with an ESRB rating and a black and white manual. A few of the classics had revision that had color manuals then we're re released with black and white manuals and ESRB ratings. I also have a variant Batman Returns cart that is well known but not mentioned here. Double Dragon also has a known classics release not on this list and i'm sure Super off Road had one too. I will get some pics up tonight.
Also, awesome pictures! The Batman Returns one is especially bizarre!

I wish I could show some examples but taz mania has a different text for the logo just like mermaid. Super Monaco GP has an organge race car on the cart front as well as a red variant. Raiden Trad also has two different colored box art with a possible label color difference.
Raiden has at least 3 variants that I'm aware of. One of the two box art releases (forget which one) had a variant with a blurb for a free pack-in turbo gamepad of some type.

The cart label differences have been discussed. They are printing variants - in Japan versus in US versus in Mexico. That was deemed to be "going too far" with respect to this thread.
Apologies on the cardboard variants in my original post; I did find the cardboard thread after posting. With regard to the labels though, pardon my ignorance, but where is the thread for this? The search function and perusing of possible relevant threads revealed nothing.

That looks like a Canadian release to me. There was a weird tendency to mix MD and Genesis branding up north.
We discussed it earlier in this thread on pages 2 and 3. Page 3, posts 3-5 are where I asked how anal we want to get in this thread about the minor printing variants and the collective responses. So I stopped posting them because no one seemed to care.
The only game we have delved deeper into with regard to printing variants is Sonic 1 NFR. That discussion is located in the thread titled "WTB Sonic 1 Canadian Variant."
Streets of Rage (Sega Classic) also has a K-A release (like Revenge of Shinobi and Sonic 1) and consistent with other Sega Classic K-A releases the manual is in black in white where as for the first release it was in color:

What does the cover of that Golden Axe look like?
I looked through eBay completed auctions and found these two listings:
Both happen to be from a Sega Classic release, though until we see more examples of the manual pop up, its impossible to say if it was packed in only with the Sega Classic release (sometimes) or where exactly its origin is.
Road Rash II has 2 releases. The first one uses the standard EA cart and includes a color manual. The second release has a regular-sized cartridge and includes a black and white manual.

That second release is the cardboard box release.
Thanks, I saw the release mixed up in all different ways and I couldn't be sure what was correct.

Yup that's it! Good work dude. Cover is the standard classics box, just like the first link.
For most variant stuff I think the thread is mostly sticking to games with boxes to help show where the variant is from. In the case of those two NRFs I believe they are from system bundles which we are tracking in the Sega System Boxes thread.
I'm going to be keeping an eye out for that Golden Axe manual in the classics box. Its just strange that one had a Mega Drive cart and the other a Genesis cart.
Don't know whether to consider this a variant or a misprint, but the Bubsy II in this auction does not have the title of the game printed on the spine:
Surely it would have to be a misprint. Why in their right mind would they release a game without the name on the spine? I wonder if there are many others the same.

I have only seen those three? I would love a TMNT or Rocket knight if they exist.

Good eye! I had no idea that an ESRB Sega Classics version of RoS existed, but it's out there. There are actually quite a few on eBay right now.
