Does anyone know what this DS accessory is and how you use it?

If I had to guess I'd say it's part of the guitar hero controller, minus the buttons, that plugged into the gba port? Possibly.

Yep, that's the piece you swap on the Guitar Hero accessory depending on if you have a regular DS or a DS Lite.
