I remember back in 07 or 08 when the Simpsons game came out on 360 (amazing game; highly recommended) and I got an achievement for "pressing start", but I pressed the A button
I wouldn't consider it the worst, but The Last Guardian has a trophy for catching Trico......... relieving herself in the game. Apparently, it's completely random, and you have to be looking at Trico when it happens. It's funny though.
That's one fine. The one that's really bad is the one for listening to every single hint in the game. That means you have to purposely sit there and do nothing for minutes at a time just to hear the game tell you what to do.

I've heard that in the PS4 Gradius re-release there were achievements involving opening a menu and other really stupid and trivial shit.
All the arcade archives branded games have those. I own quite a few like Contra, Double Dragon, Renegade, and such. 90 percent of the trophies are unlocked by scrolling through the configuration menus. it's kinda funny rattling off rapid fire bronze trophies. My favorite idiot trophy game is called My Name is Mayo. All you do is press the X button to dress up a mayonnaise jar in lingerie and funny hats for 15 minutes to get the platinum.
I earned 118 achievements for the Steam game Zup! in 0.4 hours. You get an achievement for both starting and finishing a level that usually only takes a few seconds to beat. Sometimes you earn extra achievements for random things while doing this. Then if you click around on the menu there's even more!

Going in the other direction from the OP, the "worst" for me are primarily collectibles. I can't stand how shoe-horned into games they can be, and often hundreds of them!
More recently though, and the primary reason I don't actually care about trophies at all anymore (after earning 16,000+ trophies) are games like My Name is Mayo, where you can get 100+ trophies in under an hour on two copies of the same game for $1. It just goes against everything a trophy should be - an accomplishment.

I agree that games like that ruin the sense of accomplishment. I don't understand why they would allow that game on PSN to begin with, and to let it have a platinum trophy which Sony doesn't do for every game for whatever reason. The funny thing is though, most trophy hunters seem to hate it when a game actually has hard trophies.

As far as I can tell it can randomly happen. While wondering how to do a puzzle, the trophy just popped up.

Mr Perfect - MegaMan 10, play the entire game without taking any damage
Bladder of Steel - Rock Band 2, play the entire game without pausing (about 7 hours) everybody needs a piss break every now and then
I remember that one. lol Just run up the endless song setlist, and have a blast. I messed up that achievement on RB2 by accidently pressing start.
