What is this weird looking Japanese PS1 game?

Found this at the flea market today:

I should note that large black number "SLPS 00449" printed on the front of the disc matches the holo number on the other side's inside ring of the disc.



Also any idea what the Japanese text says under, "NTSC"?

    Yeah It's a common pressed bootleg. Might only play in a modded system or an emulator.
