I have to add some new ones and agree with the earlier "Vice Project Doom" nod, as its what Bayou Billy shoulda been but with awesome Ninja Gaiden-ness.

Dick Tracy - actually was pretty good going to different buildings and fighting mobsters
Rad Racket II(hear me out on this one) - I collect so I've run across alot of these unlicensed games and out of the ones I own..this one is actually playable. i would argue it is better than straight NES tennis.
Monster Party - Im not sure how it holds up now but when i was a kid I loved it and I must have been the only one who ever rented it. lol

I played Technos Ice Hockey on an emulator once. It was okay. As a fan of Crash 'n the Boys, I didn't feel it was quite up to that standard, but it was fun for a bit. I especially liked it when you pounded your opponents enough to become enraged; their facial expressions are hilarious!
I'd like to nominate another kunio title in disguise, NES World Cup. It was just re-released on DS as River City Soccer Hooligans within the last 6months. It's a damn fun game giving orders as the captain and being able to literally kick the crap out of someone -- who needs red cards.
No doubt it is. I bet it's more in line with that DS River City Soccer Hooligans on shelves now, probably is a port of that vs World Cup if it has more beat downs in it.

I love guerilla war, i remember when i was 7 or 8 years old my cousin would come over and we would play it all day. it's been a long time since i've played it though, might pop it in tomorrow and see if it still holds up.
I played Bucky O'hare the other day - and it was unexpected funny.
My girlfriend and I were completly blown away by its creative ideas, leveldesign, the awesome graphics and the overall fun to play. The characters are cute too!
We loved to play it, thou we learned a lot of the leveldesign by doing (=failing ^^), but this was part of the experience. bucky is unforgiving

That game is tough. Unlimited continues as well as passwords make it somewhat forgiving, since you have as much time as you need to get used to how the levels are set up and whatnot, but it's still harder than NGD in a gay bar
I played only like 10-15min into that game before going through some swampy area with logs and stuff for platforms, it didn't seem that evil, perhaps it picks up heavily later on? I always thought Bucky was this stupid tv cartoon until some years later when I learned of it's harder core print version (like TMNT started out as too.)
I'd say Rygar, Adventure Island(not sure if it's really underrated on this forum), 1943, and Batman Return of the Joker(I think it's a damn fun platformer.)

Kabuki Quantum Fighter - Fantastic Ninja Gaiden clone with a horrifying twisted take on Tron plus a Japanese Irish.
Mighty Final Fight - I'm surprised more Final Fight fans don't know this exists on the NES. Nice one-two combo of RPG and beat 'em ups. Though it's cheaper to get Capcom Classics Mini Mix for GBA.
I second blockman's thing on Mighty Final Fight. It's one of those games I owned years ago and was forced to sell, love to get it back but not loving what people ask for it. It is easily in the top or damn near it spot for a beat em up of that style on the system. Unlike the stupid SNES game, you can use all 3 arcade characters too.

That's what I really disliked about it, that and Ikari Warriors 3. There's just no challenge there at all, basically, you can just camp in front of your television til you win or get bored with it. It's really a shame because Guerilla War is a very sound game otherwise but the lack of challenge kills it.
I would have to say overall, Heavy Barrel is a much better shooter and is equally, if not more, underrated.
